The information here may be of help as you enroll in your benefits.

Enrollment

If you meet the eligibility criteria, then you’re initially eligible for benefits:

  • Office hourly and salaried employees: on your date of hire
  • Production employees: when you’ve completed 60 days of employment 

You must make your enrollment elections within 30 days after your date of hire. If you don’t enroll, you’ll automatically receive limited benefits as follows:

  • Life and Accidental Death and Dismemberment (Life and AD&D) Insurance
  • Short-Term Disability (STD)
  • Employee Assistance Program (EAP)

Keep in mind that if you don’t elect Long-Term Disability and/or Optional Life Insurance during your initial enrollment period, you may elect the benefits later, but you’ll be required to provide Evidence of Insurability (EOI) for review and approval by the insurance carrier.

PLEASE NOTE:

There’s an additional $50 monthly surcharge for medical coverage if you’re covering your spouse who has coverage available through his/her employer.

Changing Benefit Elections/Qualifying Changes in Status

During Open Enrollment, you can change coverages for most benefits for the next plan year. After Open Enrollment, benefit changes can only be made if you’ve experienced a qualified change in status. Qualified changes in status include, but aren’t limited to the following changes:

  • Marital status (marriage, death, divorce)*
  • Number of dependents (birth, death, adoption, eligibility status, child support order)
  • Employment status for you or your spouse (i.e., spouse starting or terminating employment, leave of absence for you or your spouse, transition from full-time to part-time status or vice versa for you or your spouse)
  • Medicare or Medicaid coverage for you or your dependents

Generally, changes in your coverage elections must be made within 30 days of the qualified change in status.

*IMPORTANT NOTE: In the case of a divorce, covered former spouses must be removed from the plan within 30 days.
